    Ilda Vitor Maciel of Rio de Janiero, an...An 88-year-old Brazilian woman entered the hospital in 2012 with a stroke. Hospital workers injected her with soup instead of putting it in her feeding tube, causing her to die from a pulmonary embolism.

    In 2004, a man named Richard Clare fought with Timothy Magee over a cell phone in Hertfordshire, UK. Clare annoyed Magee and hit him on the back of the head with a dessert spoon lying on a table.

    The spoon hit an artery, causing fatal bleeding between Clare's skull and brain. Clare avoided punishment for the death but received seven days in jail for taking the phone that started the fight.

    Seven years later, Clare upgraded his tools and killed someone with an axe, earning a 27-year prison sentence.
