Marvel Studios can't always stay true to the comics when bringing characters to life. Tony Stark, for example, is almost identical to his print version, but others, like Yondu, have been altered. Take Helmut Zemo, who isn't a fascistic German baron like in the comics. Drax, from Guardians of the Galaxy, isn't a reincarnated saxophone player either. Anthony Mackie's Falcon can't telepathically communicate with birds, unlike his comic counterpart. Bucky Barnes wasn't a teenager in Captain America: The First Avenger, another deviation from the source material. These changes and more have been made to various Marvel Comics characters. Be warned: spoilers for many Marvel Cinematic Universe films are ahead, so proceed with caution.

Later, they were separated and told the other had died. In honor of their lost spouses, Alexei and Natasha took the codenames Red Guardian and Black Widow.

The former spouses wouldn't meet again until the Black Widow defected to the Avengers, causing conflict over an invasion of the United States. The Red Guardian sought to test himself against Captain America but gave his life saving Steve Rogers when his PRC allies tried to assassinate him.
The only real similarities between comic book Alexei and the MCU version are a name, a family tie to the Black Widow, and a desire to beat up Captain America with his basic superstrength. The MCU's Alexei was a Soviet operative before meeting Natasha, whom he raised as his daughter in a temporary sleeper cell family. This Red Guardian, defined by the silliness of a sitcom dad, seemed unable to become a supervillain or clash with Natasha.
